We are providing the forms below in Adobe Acrobat format as well as Microsoft Word format.  If you have any trouble downloading or printing the forms, please contact the Center.

Severe Allergies requiring EpiPen administration: If your child has a severe allergy that could require the use of an EpiPen, please visit our allergies page, fill out the forms, and file them with the Center.

Asthma:  If your child has asthma, please visit our asthma page, fill out the three forms, and file them with the Center.

Sunscreen:  Please read the sunscreen instructions and fill out the medication provider checklist form and and the sunscreen authorization form.  (Sunscreen documents in Word format:  instructions, checklist and authorization.)

Diaper cream and ointment:  Please read the diaper cream instructions and fill out the medication provider checklist form and the diaper cream authorization.  (Diaper documents in Word format:  instructions, checklist and authorization.)
